球員須知 General Guidelines For Players

1. 球員必須穿上整套冰球裝備。裝備必須符合 SCIHL 認可標準
   Players should wear full hockey equipment according to the SCIHL equipment standards.
2. 球員應在球賽開始前三十分鐘到達溜冰場。
   Players are required to arrive 30 minutes prior to event for preparation.
3. 當黑色暴雨警告或颱風訊號(八號或以上)懸掛時，活動將會除消; 一切改期事宜將另行通知。球員如對活動時間有任何疑問。
   Games will be cancelled when black rainstorm or typhoon signal no.8 or above is hoisted. Subsequent arrangements will be notified for the reschedule date(s).

本人同意參加上述活動，且願意遵守 貴學院之課程或活動及規則。 在此聲明本人健康及體能良好，適宜參加上述活動。如因本人疏忽、健康或體能欠佳，引致參加這項活動、使用場地、器材或設施而傷亡或財物損失， SCIHL 及 SCIHL 溜冰學院無須負責。本人同意授權予 貴學院及傳媒在不需經本人審查而可使用敝子弟之肖像、姓名、聲線及個人資料作活動及推廣之用。
I hereby declare that I am healthy, physically fit and suitable to participate in the activity and agree to abide by all rules and regulations of the SCIHL. SCIHL shall not be liable for any injury, death, loss or damage I may suffer in this activity, use of venue, equipment and facilities due to my negligence or inadequacy in health and fitness. I agree to authorize SCIHL to use my portrait, voice and personal information as the purpose of promotion without further examination.

1. 請於報名前先細閱報名須知。
   Please read carefully the Enrolment Information before your enrolment.
2. 鑑於所有運動均帶有危險，所以有意報名之參加者或其家長應先自行評估其風險。
   Since accidents may occur during all recreational activities, interested parties should therefore take the above into full consideration before enrolment.
3. 個人資料收集聲明:此報名表格上所提供的個人資料，將用作閣下/ 貴子弟參加有關活動事宜。
   Personal data collection statement: The personal data provided on this form will be used for the purpose of procession your / your child application for the stated event.
4. 此表格擬訂以中文本及英文本。表格的中文本與英文本存有差異時，一切將以英文本為準。
   This Agreement has been drawn up in both Chinese and English languages and both texts are valid. In the event of any divergence between the two texts, however, the English text shall be applied.
